Hx: Complete sacral fracture dx on intake with radiographs, currently on simbadol SQ SID and cage rest S: eating well, urinating in litter box on own, seems comfortable with current simbadol dose. O: When removed from cage and placed upright on floor was very reluctant to standing on own on tile floor. Arched back and lept into the cage. Holding LHL up but using RHL. FLs are normal. Is able to ambulate minimally and uriante in litter box on his own. Rest of PE WNL P: recheck tomorrow, continue on current simbadol dose

KNOWN HISTORY: Mimi was brought in as a stray, so there is no information on her behavior in her previous home. The finder reports she was easy to handle. MEDICAL BEHAVIOR: Observed Behavior – Allowed all handling ENRICHMENT NOTES: 07/30/17 Sitting in center of kennel, eyes dilated and ears forward. Makes eye contact when spoken to, then looks away. Allows petting along head and body while rolling over onto side and licking his lips. A bit nervous, but appreciates attention. May be uncomfortable due to injury. 07/31/17 Lying laterally towards back of kennel. He licks his lips as I reach forward and tolerates petting along his head and body while shifting his weight and continue to lip lick. Ears remain forward. May be uncomfortable due to injury. EVALUATION: Cage Condition: Cage is slightly re-arranged Reaction to assessor: Mimi was resting, but she lifts her head up and looks at the assessor with relaxed eyes. Reaction when softly spoken to: Mimi leans on her side and stretches her paws out. Reaction to cage door opening: Mimi remains soft and relaxed. Reaction to touch: Mimi sniffs the assessor’s hand and accepts petting. She leans on her side, begins to purr, and starts to knead. When petting stops, she shifts her body closer to the front. Reaction to being picked up: Did not attempt due to injury. ACTIVITY LEVEL: Cannot evaluate VOCAL: Quiet CHARACTER TYPE: Shy, Sweet, Calm BEHAVIOR SUMMARY: Average Please note that this cat is being treated for a medical condition at the time of evaluation. It is difficult to determine at this time how the medical condition may be affecting the behavior. DVM Intake Exam 7/29/17 Estimated age: ~5yrs, male Microchip noted on Intake? No History : Stray, reportedly taken in by a family about a week ago, 3 days ago was found injured outside apartment Subjective: Observed Behavior – QAR. ~5% dehydration. Allowed all handling Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = WNL BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate dental tartar/gingivitis, pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Able to stand on own, sits down if set on floor, deep pain and motor in all limbs, normal anal tone, no tail tone, no palpable fractire, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Injury to hind end Plan: Radiographs pending, simbadol 0.58ml SQ given Prognosis: Open depending on extent of injuries SURGERY: Temporary waiver due to injuries Radiographs: SQ emphysema along caudomedial right thigh, long bones intact, complete fracture at cranial aspect of sacrum. Poor prognosis
