DVM: 1431 DVM Intake Exam Estimated age: 2 years Microchip noted on Intake? no History : surrender Subjective:BAR, hydrated Observed Behavior – timid, but did well with gentle restraint for exam and treatments. Evidence of Cruelty seen – no Evidence of Trauma seen -no Objective P = 120 R = 20 BCS: 5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: negative oral. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female spayed, no mgts or dc MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: in good health Prognosis: excellent Plan: continue to monitor at ACC SURGERY:already altered

KNOWN HISTORY: Milly is curious and around strangers and comes forward to solicits attention. She’s lived with children and was relaxed and affectionate with them. She lived with a female cat and was relaxed and playful and relaxed. Her owner describes her as mellow and sometimes affectionate. MEDICAL BEHAVIOR: Timid, but did well with gentle restraint for exam and treatments. Cage Condition: No change Reaction to assessor: Milly was crouched in her litter box near the back of the kennel. Reaction when softly spoken to: Milly lip licks and lowers her head. Reaction to cage door opening: Milly remains motionless. Reaction to touch: Milly appears hesitant and her eyes widen when the assessor approaches her. She closes her eyes when touched then leans in, allowing petting and rubs on her head and cheeks. When the assessor pet her along her back she quickly whipped around and nipped. She starts to relax then rolls around and gives head butts when she comfortable. ACTIVITY LEVEL: Mellow VOCAL: Quiet CHARACTER TYPE: Sweet Timid POTENTIAL CHALLENGES: None BEHAVIOR DETERMINATION: Experience Behavior Asilomar TM – Treatable-Manageable RECOMMENDATIONS: None Recommendations comments: Milly may be a little more independent, and may need time to warm up to her new home. We recommend that this cat go to a home with experienced cat parents.
