Medical Summary DVM Intake Exam Estimated age: 4 weeks. all kitten teeth present Microchip noted on Intake? neg on intake History : stray Subjective: BARH, allows handling Objective BCS: 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: all kitten teeth present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. full belly U/G: FI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: grossly normal Assessment: 4 WK FI DSH 1. underweight and underage for scheduling spay Plan 1, offered food – no interest when just placed in front of her. when force feeding (put in face) she will chew and eat 2. foster care Prognosis: excellent SURGERY: Temporary waiver due to underage/underweigh
