*SAFE* 01/29/16 – Meet beautiful calico girl Mildred. This sweetheart came in as a stray with wounds on her paws and is in need of a warm cozy and loving new home where she can recover. Even though her 2 front legs are wrapped up she still tries to roll over for a belly rub.Pull this wonderful girl from MACC today!

initial exam
microchip scan negative
approx 2 year old female DSH cat
traumatic wounds to upper and lower lips and the distal aspect of both right and left lfront legs. the wounds on the front legs have granulation tissue forming but were covered in a layer of necrotic skin tissue – cleaned during exam with dilute chlorohexidine solution and gauze.
mandible (lower jaw) is dislocated or fractured – no instability palpated, but lower jaw is shifted to the left of center. no hard or soft palate fracture. still able to eat.
heart and lungs WNL on auscultation
underweight (BCS 2/9)
amb x 4, no neuro deficits noted
no URI signs
clean ears, no ear mites
no fleas seen
very good appetite
plan:
gave rabies vaccine SQ over right hind leg
gave FVRCP vaccine SQ over right front leg
gave 0.5 ml pyrantel PO
applied activyl topically
soaked front leg wounds in warm dilute chlorohexidine solution and gently debrided dead skin off
start amoxi/clav 0.8 ml PO BID x 14 days
start bup 0.09 ml BID x 7 days
needs new hope placement – needs wound management and x-rays of jaw +/- surgery for jaw
prognosis: fair to good with appropriate treatment
Re Exam:
Hx: Wounds to upper lip and carpi, mandible mis-alignment
S: Tolerates all handling and wound cleaning/bandaging
O: BAR-H, BCS 4/9, MMs pink and moist
EENT: Ulceration on middle 1/3 of upper lip. Mandible shifted to the left, no difficulty opening or closing jaw, no crepitus at TMJs. Left maxillary canine is fractured about 2/3 of the way down the crown. No discharge OU, AU, nose. Clean teeth.
PLNs: Not enlarged.
H/L: NSR, NMA. Eupnic, quiet lung sounds.
Abd: Soft, no pain on palpation, no masses palpated
M/S/I: Amb x4. Right front leg: swollen carpus, large scab (2-3 cm dia) over 0.5 cm deep divot on dorsal carpus, mild amount of pink purulent discharge, some crusting at nails. Left front leg: linear abrasion and ulceration diagonally on palmar aspect of carpus (1 cm wide x 4-5 cm long), some crusting at nails. Bleeding from base of 2nd or 3rd toenail after manipulation.
UG: Female
A:
1. Upper lip ulceration – R/O traumatic vs. eosinophilic infiltration (rodent ulcer) vs. cleft lip vs. other
2. Mandible shifted to left – R/O healed fracture vs. other
3. Carpal trauma, possible abscessation in right carpus
4. Underweight
Short-term prognosis: Good to fair
P:
1. Clipped, cleaned and bandaged both carpi
2. Continue abx and daily cleaning and bandage change
Enrichment:
Wounded front legs, dislocated jaw. Calm despite injury, both front legs in casts. Lay in place and leaned into pets.
