DVM Intake Exam Estimated age: 4-5 weeks based on weight, dentition and overall appearance Microchip noted on Intake? Scanned negative History : Stray Subjective: Sleeping prior to approach to kennel. Once woken up, pt was very social and walked around (despite not being able to see), purred and allowed all handling. Objective BAR-H, MMs pink and moist, BCS 2/9 EENT: Severely swollen conjunctiva, severe purulent discharge OU. Suspect severe corneal ulcers +/- ruptured eyes. Crusty purulent nasal discharge. Ears clean. Oral Exam: Deciduous teeth – canines still erupting, many PMs in place H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact kitten MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Emaciated 2. URI and conjunctivitis with possible severe corneal ulcers/rupture Plan: 1. Amoxiclav 0.1 ml PO BID x10 days 2. Erythromycin OU BID x10 days 3. Monitor appetite and weight Prognosis: Fair. Pt may have limited vision once conjunctivitis resolves. SURGERY: Too young/too small, no waiver needed 1088
