Medical Summary DVM Intake Exam Estimated age: ~4-5 years Microchip noted on Intake? positive; 985112006787585 History : stray was allegedly HBC 2 days ago and finder brought in today. Has not moved from spot since being HBC Subjective: QAR, ~5-6% dehydrated Observed Behavior – hissed when approached, unable to assess behavior due to illness Evidence of Cruelty seen – no Evidence of Trauma seen – yes Objective P = wnl R = increased rate and effort BCS 5/9 EENT: OS is swollen shut with frank bloody d/c-globe is ruptured, OD wnl no d/c, ears clean, no nasal discharge noted Oral Exam: moderate dental disease with open mouth labored breathing; swollen soft tissue around chin. Hard palate is split PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, normal lung sound on left, very harsh lung sounds on the right ABD: Non painful, no masses palpated U/G: MN MSI: unable to walk or stand but no pain elicited on ortho exam, no fractures or swelling of soft tissue noted, present withdrawal and deep pain, maggots and maggot eggs noted especially interdigitally, LFL paw is very swollen and painful with avulsion of 2nd digit and maggots in wound, no masses noted, unkempt haircoat with plant debris in coat CNS: mentation appropriate but dull; mild positional horizontal nystagmus, decreased direct PLR on right Assessment: Dyspnea r/o HBC Split hard palate Maggots Ruptured globe OS Positional horizontal nystagmus Decrease direct PLR OD Dull mentation Dehydrated Plan: Continue to monitor while at BACC in medical, reassess daily for progress Pulse ox 95% 2 view CXR-diffuse moderate bronchial pattern, no pneumothorax noted 2 view LFL rads Butorphanol 0.2mg/kg IV given for sedation Simbadol 0.24mg/kg SQ SID x3d Place IVC for IVF today 90ml/kg/day LRS CRI Soak feet in dilute chlorhex Baytril 5mg/kg IV SID Prognosis: poor SURGERY: temporary waiver for surgery due to condition
