DVM Intake Exam Estimated age: intake memo says cat was given to owner as a gift and reported to be 6yo. Cat appears to be more like 10-15 years old (moderate-severe dental dz, diffuse muscle atophy, iris atophy) Microchip noted on Intake? no History : O/S due to moving to place that does not allow pets Subjective: Cat is Q/BAR Observed Behavior – Is quiet, but observing environment and allows all handling with minimal restraint. Allows all body parts to be touched. Was hissing during blood draw, but quickly recovered and did not escalate to swatting, etc. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = wnl R = wnl BCS 3/9 EENT: Appears to have iris atrophy OU, ears- mild amount ceruminous debris, no nasal discharge noted Oral Exam: Grade 3+/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: bladder is large, but soft MSI: Ambulatory x 4, diffuse muscle atrophy, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Geriatric underlying (muscle atrophy) DDx: Hyperthyroidism, CKD, Neoplasia, Open; Moderate-Severe dental dz Plan: Recommed rescue placement or with adopter willing to take on geriatric cat that may have underlying dz. Recommend bloodwork and dental on placement. Prognosis: Fair-Good SURGERY: Already neutered

05/28/17 12:28 Midnight is a 6 year old neutered black and white DSH mix; he came to his family as a gift and is being surrendered due to the owner not being able to have pets in the home. He has no health issues or injures and has no vet history. Around strangers Midnight is friendly and allows them to held him and pick him up. He has lived with a 3 year old child and allows the child to pet him with no issue. He has never been around other animals and has no bite history. Midnight does like to scratch on the couch, and will cry during car rides. He enjoys being brushed and picked up but he will struggle to go into the carrier. Midnight was described as friendly, affectionate, playful with a high activity level. Midnight is feed dry and wet food twice a day. He is litter box trained and uses clumping litter with an uncovered box. Upon intake Midnight tried to flee but allowed the counselor to pick him up and collar with no issue, very activty during photo and had to be scruffed.

Behavior toward strangers: Friendly and allows them to pick her up Behavior toward children: Allows the child to pet him with no issue Behavior toward cats: Unknown Behavior toward dogs: Unknown Bite or Scratch history: None Litter box training: Yes Energy level/descriptors: Friendly, affectionate, playful with a high activity level Other notes: He enjoys being brushed and picked up but he will struggle to go into the carrier. MEDICAL BEHAVIOR: Is quiet, but observing environment and allows all handling with minimal restraint. Allows all body parts to be touched. Was hissing during blood draw, but quickly recovered and did not escalate to swatting ENRICHMENT NOTES: 05/29/17 Sitting towards center of kennel crouched with ears forward. He comes forward to smell my hand and allows petting along his head and body, leaning in softly. He comes forward with continued petting and begins to eat his food. Calm senior cat – allowed all petting for duration of this session. Doing well! EVALUATION: Cage Condition: No change Reaction to assessor: Midnight was relaxed at the back then comes forward to greet when the assessor approaches. Reaction when softly spoken to: Midnight sniffs the assessor through the door and looks around at her surroundings. Reaction to cage door opening: Midnight is calm and relaxed Reaction to touch: Midnight brushes up against the assessor’s body and allows petting all over. He’s social, friendly and is constantly soliciting attention. Reaction to being picked up: Midnight is calm, relaxed and has a soft body when held. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Sweet, Affectionate BEHAVIOR SUMMARY: Beginner Midnight interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. No known history of behavioral problems. This cat can go to a beginner home.
