History : brought in by a woman who found him on the street and noted he looked sick Subjective: BAR, curious, resists restraint but allows limited handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective P = purring R = purring BCS 5/9 EENT: eyes clear, no nasal discharge AU-mild brown waxy debris Oral Exam: mm pk, sl tacky; CRT <2 sec; missing upper R canine, tartar; gingivitis PLN: No enlargements noted H/L: purring, difficult to auscultate ABD: Non painful, no masses palpated U/G: male intact, 2 normal descended testicles MSI: Ambulatory x 4, skin free of parasites, diffusely dirty/dull hair coat abrasions on bridge of nose, sm amnt of dried blood and crusting; no signs of infection CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment superficial wounds dental disease Plan rec’d dental cleaning Prognosis: good SURGERY: Okay for surgery
