SAFE 3/6/18 FIV POSITIVE – Meadow was surrendered to ACC because he was a bit wobbly. It appears that Meadow has an ear infection that our medical team is treating and unfortunately, Meadow also tested positive for FIV. Meadow was born outside in a colony of cats that his owner fed. He was a very social kitten so she brought him inside as a kitten. She kept him as an indoor-outdoor cat until now. She surrendered him because he is sick and vets weren’t able to treat him.

Hx: Otitis and FIV diagnosed yesterday. Pt started on Mometamax and given Convenia. Vet assistant was unable to give MometaMax this morning due to behavior.
S/O: Hissing, growling, backs up until at back of kennel. Unable to handle.
Pt’s right ear is drooping down significantly, and pt has a head tilt to the right.
A:
1. Otitis externa – R/O eardrum rupture and otitis media
2. FIV
Short-term prognosis: Good-fair
P:
1. Simbadol 0.6 ml SQ SID x10 days
2. Onsior 0.4 ml SQ SID x3 days
3. Sedate for ear cleaning tomorrow, consider skull rads at that time.
4. Discontinue Mometamax due to inability to medicate – consider oral antifungal +/- additional oral antibiotic
5. Ivermectin 0.13 ml SQ, repeat in 10 days

History : OS – owner noted that he was off balance and so took him to another vet prior to ACC. The client indicates the the private vet declined examination b.c of being a possible rabies suspct.
Subjective: BAR, No signs of rabies were noted on visual examination

H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Two testicle present
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities

SNAP TEST POSITIVE FIV (LEFT CIRCLE BLUE)
Assessment: FIV Positive – Otitis externa of the right ear
Prognosis: fair
FIV-infected cats should be spayed/neutered and should be confined indoors to prevent spread of FIV infection to other cats in the neighborhood and to reduce their exposure to infectious agents carried by other animals. They should be fed nutritionally complete and balanced diets, and uncooked food, such as raw meat and eggs, and unpasteurized dairy products should be avoided to minimize the risk of food-borne bacterial and parasitic infections.
Plan: Treat right eat with momentamax SID for 7 days and provide 0.5cc Convenia SQ

Behavior Notes
Meadow doesn’t mind when his owner picks him up, puts him in a carrier, or wakes him up. He enjoys being brushed. He was tense but tolerant during nail trims.
Bite history:
About 2 months ago, Meadow was being pet-sat by a man who had other cats. He scratched the man. His owner thinks the man was trying to pick him up. Afterwards, he ran and hid.
Meadow’s owner doesn’t know of him biting/scratching any other people or being bit/scratched by any animals. She reports he has a possible wound on his ear.

Meadow began stumbling/wobbling the evening of 2/25. He ate yesterday. His owner doesn’t know if he ate today. The last time he escaped and spent time outside was 2/19/18.
For a New Family to Know
Meadow followed his owner and sat next to her or in her lap. His owner tried to keep him inside but he would sometimes run out the door and comes back that evening or the following morning. He slept with his owner or on the fridge. He ate dry cat food and enjoyed tuna as a treat. He used an open litterbox with clumping litter but had been starting to spray in corners. Meadow scratched the couch initially but when his owner got him a flat cardboard scratching pad he used that instead.
