ACTIVITY LEVEL: Steady VOCAL: Quiet CHARACTER TYPE: Timid KNOWN HISTORY: Other notes: Maxima was brought into the care center as a stray with unknown history. However, during intake she allowed all handling and is calm when being picked up and held. She lays down accepts pets and scratches and head butts counselor. MEDICAL BEHAVIOR: 6/29/17- Subjective: Meows and hisses from inside the cat den – continues to meow and hiss throughout the exam. ENRICHMENT NOTES: 06/29/17 -Seems soft and relaxed, lying down on her cage bedding until the approach. She started to growl and hiss, not allowing any contact. -In den on approach, started grumbling when I got closer. Attempted treatment by medical, stayed in den, hissed and growled throughout. EVALUATION: Cage Condition: No change Reaction to assessor: Maxima remains neutral lying down in her cat den during the approach. Reaction to cage door opening: Maxima remains immobile in place. Reaction to touch: Maxima does not allow the touch, hisses and growls at the assessor’s hand to stop the interaction. BEHAVIOR SUMMARY: Maxima is displaying behaviors that preclude placement in the adoptions room and/or may require further investigation before placement in a home. The behavior department feels that placement with a New Hope Partner is the best option at this time.

Scanned negative on LVT intake History : No hx available Subjective: Meows and hisses from inside the feral den – continues to meow and hiss throughout the exam. Limited exam with pt wrapped in a towel. Objective BCS 6/9, MMs pink and moist EENT: Moderate mucoid ocular discharge OU, moderate purulent nasal discharge. Mild brown granular debris AU. Oral Exam: Severe dental tartar accumulation. PLN: Unable to palpate H/L: Unable to auscult due to behavior. Eupnic. ABD: Non painful, no masses palpated U/G: Female, presumed intact but unable to confirm by shaving abdomen due to behavior MSI: Pt moves a little slowly. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Older female cat with: 1. Dental disease 2. URI and conjunctivitis 3. Likely ear mites Plan 1. Erythromycin OU BID x10 days 2. Doxycycline 1 ml PO SID x10 days 3. Nebulize BID x5 days 4. Ivermectin 0.15 ml SQ given during exam today – repeat in 2 weeks 5. Recommend dental cleaning Prognosis: Good-fair SURGERY: Permanent waiver due to old age. 1088
