Medical Summary Sex: Male neutered Scan: Negative Estimated age: 6 years per client Fleas: No flea dirt. Hx: Has been urinating all over the house for the last few days or weeks (client not clear). Also defecated outside of the litter box this week. Pt was taken to vet and given injections. S: Friendly, allows all handling O: QAR-H, BCS 5/9, MMs pink and moist EENT: No discharge OU, AU, nose. Mild tartar and gingivitis. PLNs: Not enlarged H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male neutered. A: Inappropriate urination and defecation. R/O medical (FLUTD, UTI, stones, other) vs. behavioral (stress due to environmental change, change in litter box placement or arrangement, other) Short-term prognosis: Fair P: 1. Gave 0.85 ml pyrantel and Activyl 2-9# during exam 2. CBC, chemistry and urinalysis 3. Monitor bathroom habits in cage at ACC 1088
