Medical Summary DVM Intake Exam Estimated age: 2 years Microchip noted on Intake? negative History : stray, found on street lethargic, not eating, not drinking Subjective: Quiet, responsive but not alert. Laterally recumbent Observed Behavior – unable to assess Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = 90 F P = bradycardic, ~80 bpm R = 15 bpm BCS: 2/9 EENT: Eyes cloudy with moderate purulent d/c, chemosis, third eyelids elevated. Purulent nasal d/c with audible upper respiratory congestion. Mild brown waxy debris Oral Exam: jaw is tight and mouth is difficult to open. no oral ulcers but foul odor in mouth. Clean adult dentition PLN: No enlargements noted H/L: NSR, NMA,, bradycardic. CRT 3, pale, Lungs clear but audible upper resp congestion and increased effort to breathing. ABD: Non painful, no masses palpated U/G: F, some purulent d/c around perineum unclear of origin MSI: laterally recumbent and non-ambulatory, skin free of parasites, no masses noted, haircoat is wet and unkempt CNS: mentation inappropriate, not alert but PLRs intact and pupils of equal size. No menace but not alert enough to respond. CN intact. Pupils miotic. Assessment: Severe URI Laterally recumbent Dehydrated Emaciated Plan: CBC/Chem LRS 20ml/kg bolus followed by 150ml/kg/day CRI doxycycline 10mg/kg PO SID x10d erythromycin ou BID x10d Prognosis: poor SURGERY: Temporary waiver due to URI
