DVM Intake Exam
Estimated age: ~14-16 years old
Microchip noted on Intake? negative
History : o/s-o claims cannot take care of cat any longer
Subjective: QAR and 10% dehydrated
Observed Behavior – very sweet but also too weak to assess behavior
Evidence of Cruelty seen – yes
Evidence of Trauma seen – no
Objective
P = wnl R = eupneic BCS 1/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: moderate dental disease with fractured teeth, no oral lesions seen
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated, gas filled intestinal loops
U/G: F, unknown if spayed
MSI: Ambulatory x 4 but very weak, MCS 0/3, BCS 1/3, skin free of parasites, no masses noted, coat is soaked in diarrhea especially around perineum. Pinna, gums, tongue, and sclera are icteric
CNS: mentation appropriate – no signs of neurologic abnormalities

Geriatric
Plan: Continue to monitor while at BACC
Give LRS bolus 20ml/kg then start on 100ml/kg/day
CBC when able to draw enough blood
Chemistry-when able to draw enough blood
Parvosnap-negative
If she does not significantly improve by tomorrow, rec HE
Prognosis: Guarded
SURGERY: permanent waiver from surgery due to illness and age
