GONE 7/19/17 Poor 10 year old Mandi was found panting heavily – she was hungry and finder brought to the shelter. Mandi has a heart murmur and a small mass near her eye – and some other issues going on that will need follow up vet care. She is friendly and relaxed and would love to have a real home. Mandi is a polydactyl kitty.

History : brought in as a stray; she was noted to be friendly and relaxed with finder; on intake, she was panting heavily Subjective: BAR Observed Behavior – panting heavily but still wanted to eat Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T =102.2 P = 220 R = pant BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, CRT <2 sec; mild hypersalivation; fracture of upper L canine with poss root exposure; moderate to severe tartar/staining PLN: No enlargements noted H/L: panting with normal effort; tachycardic, SSFP; grade II/VI heart murmur ABD: soft, nonpainful, doughy; no palpable abnormalities U/G: female MSI: polydactyl, nails overgrown ambulatory x 4 mildly unthrifty hair coat diffuse muscle wasting pea sized, pigmented cutaneous mass near medial canthus OD CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Panting-suspect thyroid storm Heart murmur Emaciated Geriatric Cutaneous mass Plan CXR-2 view, mild to moderate diffuse bronchiole pattern; cardiomegaly but no obvious CHF CBC/Chem/T4-mild anemia (poss regenerative?), lymphopenia, mod-severely elevated ALT (572), elevated ALP (300), low creat; severely elevated T4 (18.3) gave 0.2 ml torb IM gave 0.12 ml lasix IM gave 5 mg methimazole PO, rec’d continuing 2.5 mg PO BID calmed down after torb but was still intermittently open mouth breathing; ate some food and drank water recheck T4, BUN/creat in 7-10 days rec’d cardiac ultasound Prognosis: fair to good with management SURGERY: permanent waiver due to age
