KNOWN HISTORY: Cubby arrived at the shelter as a stray, so we cannot speak to his behavior in a previous home. At intake, Cubby was alert and timid, allowed all handling. EVALUATION: Cage Condition: No change Reaction to assessor: Lying on blanket at back of cage, quiet, calm, unmoving, looks up at assessor briefly but otherwise keeps head and eyes down. Reaction when softly spoken to: Cubby remains quiet and motionless, but does look around, doesn’t focus on anything in particular. Reaction to cage door opening: Cubby remains quiet and motionless, but does look around, doesn’t focus on anything in particular. Reaction to touch: Cubby allows petting all over, seems a little uncomfortable, leaning away but doesn’t move. Reaction to being picked up: Cubby allows himself to be picked up and held. Remains quiet, calm while held. MEDICAL BEHAVIOR: Observed Behavior – Quiet, calm, allows all handling. ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Shy, Calm, Mellow, Docile BEHAVIOR SUMMARY: Please note that Cubby is being treated for an upper respiratory infection and oral ulcers at the time of the behavior assessment. This condition may affect some of the behaviors shown during evaluation. X__ Average Cubby is easy to handle and tolerates all petting. He may need time to warm up in his new home. This cat is showing behavior that appears suitable for an adopter with an average amount of experience.

03/02/17 History – transfer from Queens shelter, stray cat. Finder noted cat is not eating. Microchip scan neg. Subjective Observed Behavior – Quiet, calm, allows all handling. Evidence of Cruelty seen – no Evidence of Trauma seen – no. Objective P = 200 R = 16 BCS = 3/9 ORAL: Drooling, mild to mod gingivitis, mild calculus, with several oral ulcers on right buccal mucosa. Sensitive on oral exam. EENT: Eyes clear, ears clean, no nasal discharge noted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic. ABD: Non painful, no masses palpated U/G: Neutered male. MSI: Ambulatory x 4, skin free of parasites, no masses noted. Dry, unkempt haircoat. Mod decreased skin turgor. Thin body condition with mild generalized muscle wasting. CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment Medical Behavior Status = Blue Medical Status 2 C Asilomar Status T/R Fast Track? No Age est 6 years. Dehydration 5-7%. Oral ulcers – r/o calicivirus, other infectious disease, dental disease. Thin condition – r/o due to oral disease, infectious disease, parasitism. Plan 100 ml LRS SQ q 24 hours x 3 days. Buprenorphine 0.3 mg/ml 0.1 ml SQ BID x 5 days. First dose administered. Monitor appetite and hydration. Good prognosis.
