Medical Summary DVM Intake Exam Estimated age: 3 years Microchip noted on Intake? no History : Stray, no history available Subjective: BAR, attempted to jump off table during exam Observed Behavior – near-continuous growling, occasional hissing, attempts to flee, but never overtly aggressive. Could not get spay scar, Combo test, microchip. Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 8/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild tartar and gingivitis, missing incisors right maxillary quadrant PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated; significant fat pad precludes abdominal palpation U/G: female; no discharge or staining noted; could not check for spay scar MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Obese, nervous, otherwise healthy Plan: schedule for surgery in-house so can check for spay scar; Combo test/microchip at time of surgery Prognosis: excellent SURGERY: Okay for surgery
