KNOWN HISTORY: Madeline was brought in as a stray, so we cannot speak to his/her behavior in his/her previous home. Upon intake Madeline was tense and kept a low body posture. MEDICAL BEHAVIOR: very sweet but a little nervous. Easily handleable and easy to pick up. ENRICHMENT NOTES: 08/16/17 Lying in back of kennel, crouched body position. Relaxed, ears are forward. She leans in when pet along her head and body. Doing well! EVALUATION: Cage Condition: Cage recently cleaned Reaction to assessor: Madeline was resting in the back of her kennel with her eyes closed. Reaction when softly spoken to: Madeline remains motionless and continues resting, did not wake up. Reaction to cage door opening: Madeline lifted her head up and remains calm. Reaction to touch: Madeline slow blinks, rolls on her back and leans in for cheek rubs. She comes forward and head butts assessor. Reaction to being picked up: Madeline purrs and remains calm. She has a relaxed body and continues to solicit attention. ACTIVITY LEVEL: Mellow, Laid back VOCAL: Quiet CHARACTER TYPE: Sweet, Affectionate, Calm BEHAVIOR SUMMARY: Beginner Madeline interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: clean adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated, lots of gass and diarrhea palpated in colon U/G: FI, no vulvar d/c or MGTs MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Diarrhea r/o stress vs internal parasites vs other Plan: Continue to monitor while at BACC-recheck diarrhea in 5 days, if no improvement consider metronidazole and fecal Add ponazuril in addition to pyrantel Prognosis: Excellent SURGERY: Okay for surgery
