Medical Behavior Evaluation BLUE
Medical Summary DVM Intake Exam Estimated age: 3 Microchip noted on Intake? Negative History : Owner evicted. Surrendered by property manager> Housed with another cat (A110914) and a dog. Subjective: QAR. Overall good condition. Small wound on head. Observed Behavior – Difficult to pull out of carrier, cautious and a bit jumpy. Hissed when giving microchip but fine with vaccines. Evidence of Cruelty seen – none. Evidence of Trauma seen – none. Objective T = P = 200 R = BCS=5/9 EENT: Eyes clr. Ears – L ear has a few scabs and dried blood inside on mid pinna. R pinna clean. Clean in the canal of both ears. No nasal discharge noted. Oral Exam: Teeth – slight tartar on caudal teeth, minimal gingivitis. PLN: No enlargements noted H/L: NSR, NMA, MM pk, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Shaved, no spay scar seen but a bit jumpy. Slight 0.5 cm umbilical hernia seen. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat overall but 5 mm superficial dermal wound on R dorsal neck with hair dried over it. No fleas seen. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done Assessment: 3 yo Female intact DSH. Wound R neck and L ear pinna – r/o scratching himself vs scratched from other cat Mild umbilical hernia. Plan: Clip/ clean with chlorhex wound on neck. Cleaned scabs inside L ear. Placed ab oint. Re-check wound in 2 days to assess healing. No abs for now. Re-check tech check to check for spay scar. (Previously was housed with intact male.) Prognosis: Good. SURGERY: Okay for surgery
