KNOWN HISTORY: Madden was brought in as a stray, so we cannot speak to his behavior in his previous home. MEDICAL BEHAVIOR: Reluctantly allows handling. Hissed when giving microchip but fine with vaccines. Hissing and trying to scratch/ get away for venipuncture. ENRICHMENT NOTES: 05/04/17 At the front of the kennel as I approach, leaning against the kennel door. He hisses when I open the door and comes forward to head-butt my hand. He allows all petting, stepping back with a tense body every once in awhile with his tail twitching. He calms down and allows petting again with his tail raised, leaning in. Hisses when I close the kennel door. Friendly guy, but needs some more time to adjust. Sprayed feliway. EVALUATION: Cage Condition: Cage recently cleaned Reaction to assessor: Madden is very vocal and comes forward immediately to greet the assessor. Reaction when softly spoken to: Madden rolls around on his back and purrs softly as he reaches out with paws for attention. Reaction to cage door opening: Madden waits eagerly by the door, ready to interact and explore. Reaction to touch: Madden is very curious of his surroundings and tries to walk out the kennel immediately. He can be distracted easily then rubs up against the assessor for attention. He meows loudly and his tail starts to quiver when pet along his body. After a couple soft pets he goes right back to the front and tries to walk out the kennel. Reaction to being picked up: Madden was a little tense when picked up but remained calm, and allowed all handling. ACTIVITY LEVEL: Energetic VOCAL: Talkative CHARACTER TYPE: Curious, Social, Sweet BEHAVIOR SUMMARY: Average Madden interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

Mild dehydration. Overall good condition. Observed Behavior – Reluctantly allows handling. Hissed when giving microchip but fine with vaccines. Hissing and trying to scratch/ get away for venipuncture. Evidence of Cruelty seen – none. Evidence of Trauma seen – none. Objective T = P = 200 R = BCS=5/9 EENT: Eyes clr, ears cln, no nasal discharge noted. Oral Exam: Teeth – mild tartar and gingivitis on caudal teeth PLN: No enlargements noted H/L: NSR, NMA, MM pk, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact male – 2 testicles. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done Assessment 5 yo intact male DSH Mild dehydration. Plan: Feed canned and dry. Re-check hydration in 2 days. Prognosis: Excellent SURGERY: Okay for surgery
