Medical Behavior Evaluation BLUE
Medical Summary 06/01/17 DVM Intake Exam Estimated age: 6 weeks Microchip noted on Intake? No. History : Stray kitten, found in a library. No medical concerns noted. Subjective: Observed Behavior – Shy, a little tense. Allowed all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=lt pink, moist, CRT<2 s. HR=220, RR=28. BCS=4/9. ORAL: All decid. teeth present, no oral lesions. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/oorganomegaly palpated. U/G: Male, intact. MS: Ambulatory x 4, no lameness. INTEG: Full, clean haircoat. No skin lesions or masses. CNS: Mentation appropriate, no signs of neurologic abnormalities. A: Apparently healthy 6 week old kitten. P: Temporary waiver from neuter surgery due to being underage and underweight. Rec. foster. Excellent prognosis.
**** PLEASE NOTE **** KITTENS under the age of 8 weeks or weighing 2 pounds or less CAN ONLY be PULLED by a RESCUE!!!! IF INTERESTED, PLEASE contact a RESCUE ASAP!!!
