Hx: Surrendered 7/6/17, spayed 7/8/17, this morning pt is open-mouth breathing and sneezing S: Active, walks around exam table, allows all handling O: BAR-H, BCS 5/9, MMs pink and moist EENT: Serous nasal and ocular discharge. Clean ears and teeth. PLNs: Not enlarged. H/L: NSR, NMA. Sneezing regularly. Quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Female spayed, incision has no swelling, discharge or erythema. Mammary glands are still slightly enlarged but pt is not lactating. Neuro: Alert and appropriate, no sign neurological deficiencies A: URI P: Move to isolation, start doxycycline 0.6 ml PO SID x10 days, nebulization BID x3 days, 100 ml LRS SQ SID x4 days. 1088

07/06/17 15:35 Basic Info Luna Moon is a 2 year old DSh whose owner had too many pets and cold not keep her. She has never been to a vet before. Socialization Luna Moon is social around strangers and approaches them and rubs on them and solicits attention. She is not around children often but lives in a home with several other cats that she gets along with very well and is very tolerant. She has never been exposed to dogs before and has never bitten a person or animal before. Behavior Luna Moon does not scratch on furniture has no litter box issues. Luna Moon is not bothered by being bathed or picked and held. New Family Luna Moon has a medium level of activity and loves to be around her owner wherver she is in the home. She is kept solely indoors and sleeps in her cat tree. She is fed Purina wet and dry food. She has no litter box issues and has an uncovered litter box with fresh step litter. She scratches on her cat tree that is made of bark and carpet. Intake Behavior Luna moon forcefully headbutts counselor and any other object around and purs and head butts over again, she allows all handling and solicits a lot of attention. All handling for collaring, scanning and photograph were allowed by her.

EVALUATION: Cage Condition: Cage is neat Reaction to assessor: Luna Moon was meowing repeatedly, soliciting attention. Reaction when softly spoken to: Luna Moon continues to meow and climbs down from her shelf and jumps on the shelf again. Reaction to cage door opening: Luna Moon greets the assessor by the front. Reaction to touch: Luna Moon head-butts the assessor vigorously and begins to purr. She rubs her entire body on the assessor and is very active throughout the interaction. Reaction to being picked up: Luna Moon remains soft, but wiggles out of hold to continue head-butting the assessor. ACTIVITY LEVEL: Lively, Active VOCAL: Talkative CHARACTER TYPE: Social, Sweet, Insistent BEHAVIOR SUMMARY: Beginner Luna Moon interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting.
