Medical Summary DVM Intake Exam Estimated age: 4 weeks Microchip noted on Intake? not scanned History : Stray Subjective: Patient BAR, nervous but allows all handling, ate well when offered kitten food Observed Behavior – Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective BCS 5/9a=, eupneic EENT: Eyes clear, ears clean, bilateral purulent nasal discharge Oral Exam: Negative oral exam, deciduous dentition present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally, DRE not performed Assessment 4 wk FI DMH URI Underage Plan: NH placement FVRCP Frontline Pyrantel and ponazuril FIV/FELV: negative Prognosis: good SURGERY: Temporary waiver due to underage/URI
