Medical Summary DVM Intake Exam Estimated age: reported to be 12 by previous owner Microchip noted on Intake? none History : owner surrender; reported to be healthy with previous vet records available Subjective: QAR Observed Behavior – shy but very sweet! allows handling and restraint Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T =not taken P = WNL R = WNL BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; tip fracture of upper L canine with no obvious root exposure; moderate tartar and staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: reported to be spayed MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat with mild overgrooming on abdomen CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Apparently healthy geriatric Plan monitor for any signs of systemic illness (weight loss, decreased/increased appetite, pu/pd, vomiting/diarrhea) and if any are noted then rec’d CBC/chem Rec’d dental prophylaxis Prognosis: excellent SURGERY: spayed
