Medical Summary DVM Intake Exam Estimated age: 1-2 Microchip noted on Intake? n History : part of a group being removed from a construction site, all flea infested most with scabs/scars on their heads Subjective: Observed Behavior – all in group will flee but no attempts to bite etc Evidence of Cruelty seen – n Evidence of Trauma seen – old Objective P = 200 R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: minimal–seems wnl H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact female MSI: Ambulatory x 4, fleas in group. few dime sized scars on head CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: fleas, minor scabs on head Plan.ohe Prognosis: good SURGERY: Okay for surgery
