05/13/17 09:11 Basic Info: Louie A1111924 is a neutered male brown tabby & white DSH feline who was given to the owner as a gift from a friend as a kitten & is being surrendered due to moving to where pets aren’t allowed. He was last seen by a vet last year when neutered and currently has no known health issues/injuries. Socialization: Louie is friendly with strangers. He hasn’t been around other animals but was gentle, playful, relaxed/affectionate & respectful with the adults & a 5yr old in the home, however when the child’s energy became too much for Louie he would run/hide from the child. Louie hasn’t bitten/scratched any person or animal. Behavior: During car rides Louie will meow. He isn’t bothered with being placed into a carrier, being brushed or being held/pet (but it’s for short periods of time if he isn’t comfortable with you or he wants to be let go of). Louie hasn’t been disturbed while sleeping/resting. When the owner tried to trim his nails or bathe him they stated Louie will struggle to get away until you let go. Louie scratches on the wall at times & doesn’t have a scratching post. For a New Family to Know: Louie is described as friendly, affectionate, quiet/mellow & playful (at times) with a low-medium activity level. When the owner is home Louie follows them around. He’s an indoor only cat that sleeps in the dining room because it’s dark and quiet, likes to lay on the windowsill, eats wet Purina cat chow 2-3x a day (according to the owner dry cat food makes him vomit) and uses a hooded litter box with clumping litter within. Intake Behavior: Louie had a soft body, eyes and allowed all handling. He allowed to be scanned (-), collared & have his pictures taken; he’ll lean into pets as well & purr. Louie moved around quite a bit and made several attempts to jump off the backdrop table. He didn’t make much eye contact with the camera and his moving around made it difficult to get a picture of him hence the kennel picture. Louie didn’t seem to mind being picked up/held and was placed into a kennel with ease.

ACTIVITY LEVEL: Mellow VOCAL: Somewhat chatty CHARACTER TYPE: Shy, Curious KNOWN HISTORY: Lived Indoors Previously lived with: an adult and 5 year old Behavior toward strangers: friendly Behavior toward children: gentle, playful, relaxed/affectionate & respectful Litter box training: yes Other notes: described as friendly, affectionate, quiet/mellow & playful (at times) with a low-medium activity level. MEDICAL BEHAVIOR: 5/14/17-Subjective: Growling in the carrier. Did not allow any handling EVALUATION: Cage Condition: No change Reaction to assessor: Louie looks neutral, sitting upright by the front when approached. Reaction to cage door opening: Louie solicits attention. Reaction to touch: Louie leans into the assessor’s hand, but becomes anxious with the touch and slowly retreats while he randomly low growls as he seeks more attention. BEHAVIOR SUMMARY: Louie may be a little more independent, and may need time to warm up to his new home. We recommend that this cat go to a home with experienced cat parents.

DVM Intake Exam Estimated age: Reported 2 years, exam is consistent with this Microchip noted on Intake? Placed yesterday History : Neutered last year, no other medical hx given Subjective: Growling in the carrier. Did not allow any handling – hissing and growling and trying to run. Escaped and ran away – put in Freeman net for exam and then given Dexdomitor 0.4 ml IM to facilitate exam and CT. Objective BCS 5/9, MMs pink and moist EENT: Eyes clear. Very mild serous nasal discharge and small nasal planum ulcerations. AD moderate black discharge – AS clean. Oral Exam: Clean teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male neutered MSI: Tail amputated about 1/3 of the way down – no scarring or alopecia. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment 1. Otitis – likely ear mites 2. Tail amp – done surgically as it has healed very well 3. Mild URI Plan 1. Cleaned AD and administered ivermectin 0.15 ml SQ during exam – repeat in 2 weeks 2. Swabbed for resp pathogens 3. Doxycycline 1 ml PO SID x10 days Prognosis: Good Reversed with 0.4 ml Antisedan IM 1088
