Eating wet food on own Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective T= 100.3F P = WNL R = WNL BCS 5/9 EENT: Eyes clear, conjunctivitis OU, ears clean, moderate mucoid nasal discharge noted Oral Exam: Pink mm, teeth clean PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, thin hair coat on face, ~1.5cm area of alopecia on right front antebrachium, alopecia at right hind him CNS: mentation appropriate – no signs of neurologic abnormalities Woods lamp positive on face and limbs Assessment: Underage, URI, conjunctivitis, ringworm positive Plan: Rec doxycycline 0.15ml PO q24 x 10 days, erythromycin OU q12 x 10 days, lime sulfur dip 2x/week, itrafungol 0.3ml PO q24 x 21 days DTM taken Prognosis: Good with appropriate treatment SURGERY: Temporary waiver due to underage, URI, ringworm
