Yes #0A13481013 History : Seized Subjective: QAR Observed Behavior – Hiding at back of carrier, nervous but easily extracted from cage. Completed exam and vaccines, drawing blood with no difficulty. When trying to give pyrantel escalated and tried to bite. Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective T = P = 160 R = 44 BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted. Small notch in pinna of left ear (AS) Oral Exam: Moderate tartar, pale pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male, no testicles in scrotal sac, presumed neutered. MSI: Ambulatory x 4, skin free of parasites, Unkempt dull haircoat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Very thin, poor bcs, unkempt haircoat. Plan: Recommend CBC/Chemistry/T4 levels to rule out hyperthyroidism , renal disease, other. Prognosis: Fair-Good
