Medical Summary DVM Intake Exam Estimated age: 4 weeks Microchip noted on Intake? negative History : stray Subjective: BARH Observed Behavior: scared in the cage but once out of the cage and being handled was friendly and tolerated all medical handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: no oral lesions noted, kitten dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated, distended U/G: FI MSI: Ambulatory x 4, live fleas and flea dirt noted, no masses noted, unkempt haircoat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Live fleas Underage Plan: CTM while at BACC Prognosis:good SURGERY: Temporary waiver due to underage/underweight
