Medical Summary 06/04/17 DVM Intake Exam Estimated age: 1 yr Microchip noted on Intake? No. History : Stray cat, finder noted that she seems wobbly. Subjective: Observed Behavior – Quiet, a little timid but calm. Allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=220, RR=20. BCS=5/9. ORAL: All adult teeth erupted, excellent dental condition, no oral lesions. EENT: No oculonasal discharge, no sneezing. H/L: Lungs clear, no murmurs/arrhythmias. PLN: No enlargements noted ABD: Non painful, no masses/organomegaly palpated. U/G: Female, unspayed. MS: Ambulatory x 4, no lameness. INTEG: Full, clean haircoat. Skin free of parasites, no masses noted. CNS: Mentation appropriate. Wide based stance with mild to mod. ataxia, occassionally will stumble and fall but able to get back up easily. Minor head tremor. A: Approx. 1 yr old cat with head tremors, ataxia – r/o cerebellar hypoplasia. P: Permanent waiver from OVH surgery due to neurologic disease. Good prognosis with appropriate management.
