07/10/17 17:21 Basic information Littelfoot is a 4 month old black and white DSH with a possible eye infection. Littlefoot was found wandering loose outside and allowed finder to pick him. Finder described Littelfoot as firmly affectionate and shy with a medium activity level. Littelfoot was not introduced to children cats or other dogs while with the finder. Littlefoot does not appear litter box trained. Little foot enjoyed playing with balls and was relaxed when being picked up and held. Little foot wasn’t bothered when being placed in a carrier. On bus ride Littlefoot cried. Little foot does not have a bite or scratch history. Behaviors during intake During intake Littlefoot was calm and quiet. He allowed to be scanned and collared with a loose body. Littlefoot enjoyed being pet and would begin to purr. Littlefoot allowed all handling.

ACTIVITY LEVEL: Lively VOCAL: Somewhat chatty CHARACTER TYPE: Social, Sweet MEDICAL BEHAVIOR: 7/12/17- Subjective: Purrs, leans into handler ENRICHMENT NOTES: 7/11/17- At the front, eating wet food, soft eyes and body. Leaned and arched into pets, purred. Sat calmly when held. EVALUATION: Cage Condition: No change Reaction to assessor: Littlefoot immediately comes soliciting at the front of the cage. Reaction to door opening: Littlefoot remains standing at the front of the cage, relaxed body posture, soliciting attention. Reaction to touch: Littlefoot head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to Being Picked up: Allows the pickup and remains calm. BEHAVIOR SUMMARY: Beginner Littlefoot interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

DVM Intake Exam Estimated age: 12-16 weeks old based on dentition Microchip noted on Intake? Scanned negative on intake – MC placed History : Stray kitten, appears to have URI and patchy hair loss which might be ringworm Subjective: Purrs, leans into handler. Energetic – moves around and climbs out of restraint. Objective BAR, BCS 4/9, MMs pink and moist EENT: Mild mucoid discharge OU. Clean ears. Large amount of purulent nasal discharge. Patchy hypotrichosis on lateral aspects of both ears. Oral Exam: Clean deciduous teeth, two pairs of maxillary canine teeth appear to be adult teeth. PLN: No enlargements noted H/L: NSR, NMA Referred upper airway sounds, quiet lung sounds. ABD: Non painful, no masses palpated U/G: Male intact kitten, 2 testicles soft/symmetrical MSI: Ambulatory x 4, small alopecic patches on ears but no other possible ringworm spots. No masses noted. CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally. Wood’s lamp – small amounts of green fluorescence within the round patches on the ears. No fluorescence on the rest of the body – including feet, abdomen etc. Assessment: 1. URI and conjunctivitis 2. Possible ringworm Plan: 1. Performed DTM culture 2. Lime dip tonight and q3-4 days, or stop if this first DTM culture comes back negative 3. Itrafungol0.8 ml PO SID x21 days or stop if this first DTM culture comes back negative 4. Erythromycin OU BID s10 days 5. Doxycycline 0.3 ml PO SID x10 days 6. Nebulize BID x5 days Prognosis: Good SURGERY: Temporary waiver due to: URI and possible RINGWORM 1088
