Medical Summary 05/17/17 DVM Intake Exam Estimated age: 5 weeks Microchip noted on Intake? No. History : Litter of 4 stray kittens, no health concerns noted. Subjective: Observed Behavior – Very shy, hiding in back of cage trembling, hisses when approched. Covered with small towel to pick up and restrain, allowed all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR>240, RR=20. BCS=5/9. ORAL: All decisuous teeth erupted, no oral lesions. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, unspayed. MSI: Ambulatory x 4, no lameness. Full, clean haircoat, no skin lesions or fleas. CNS: Mentation appropriate – no signs of neurologic abnormalities. A:Apparently healthy 5 week old kitten. P: Temporary waiver from OVH surgery due to young age. Excellent prognosis. Rec. foster placement for socialization.
