Medical Summary DVM Intake Exam Estimated age: 4 weeks Microchip noted on Intake? n History : stray Subjective: extensive wounds on left caudal thigh, LH paw back and base of tail. Wounds are dry and all tissue and limbs vital. small defect OD eyelid. kitten ate when offered. I would guess these are thermal burns, but chemical or trauma are possible too Observed Behavior – scared but relaxes with handling Evidence of Cruelty seen – n Evidence of Trauma seen – y Objective P = 200 R = wnl BCS 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: decid teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male MSI: Ambulatory x 4, skin free of parasites, no masses noted, CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: wounds , underweight Plan SSD to wounds bid, amoxiclav 44mg/ml 0.1ml bid x 10d gave 15cc lrs sq Prognosis: fair/guarded SURGERY: Temporary waiver due to age
