Medical Summary DVM Intake Exam Estimated age: 1-2 years based on overall appearance Microchip noted on Intake? Scanned negative History : Came in with 4 kittens. Two kittens had an umbilical congenital defect with maggot infestation and were euthanized. One kitten had a partial-thickness umbilical defect with no maggots. One kitten had no umbilical abnormalities. Subjective: Wide-eyed, relaxed body, doesn’t move, allows all handling Objective BAR-H, BCS 5/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Clean teeth PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Soft, non painful, no masses palpated U/G: Female, lactating. No vulvar discharge. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Likely carries the gene for a congenital abnormality at the umbilicus 2. Lactating – has two surviving 5-7 day old kittens Plan: Nurse kittens x8 weeks, then wean kittens and spay this patient. Do not recommend future breeding due to congenital deformities of this litter. Prognosis: Good SURGERY: Temporary waiver due to 1088
