History : found at JFK terminal; finder was interested in adopting so took her to her regular vet and vet was concerned about poss ringworm Subjective:BAR Observed Behavior – very sweet and interactive, allows all handling; great appetite Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 5/9 EENT: OU-periocular alopecia, crusting, focal ulceration; focal areas that are wood’s lamp positive; elevated 3rd eyelid mild ulceration of nasal planum, no active discharge or sneezing noted AD-notch out of tip of ear Oral Exam:mm pk, moist; CRT <2 sec; minimal tartar/staining PLN: No enlargements noted H/L: grade II squeaky heart murmur ABD: Non painful, no masses palpated U/G: female MSI: Ambulatory x 4, skin free of parasites, no masses noted periocular alopecia/dermatitis-see ocular notes thin hair/alopecia along dorsum-small area of crusting/dandruff that is wood’s lamp positive CNS: mentation appropriate – no signs of neurologic abnormalities Rectal:not performed Assessment Poss ringworm Conjunctivitis Plan DTM culture Erythromycin OU BID x 10 days Convenia 0.3 ml SQ further treatment based on DTM culture results Prognosis: good SURGERY: temp waiver due to poss ringworm
