Medical Summary 05/24/17 DVM Intake Exam Estimated age: 1 yr. Microchip noted on Intake? No. History : Brought in by owner, no medical concerns noted. Subjective: Observed Behavior – Very shy, tense, Wrapped in towel for exam and treatments, allowed all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=220, RR=28. BCS=4/9. ORAL: All adult teeth present, no oral lesions. EENT: Eyes clear, no nasal discharge noted. Mod. crusty black debris and erythema AU – ear mite pos. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, no spay scar found. MSI: Ambulatory x 4, no lameness. Skin free of parasites, no masses noted, healthy hair coat. CNS: Mentation appropriate. A: Young adult cat with ear mites. P: OK for OVH surgery. Cleaned AU, applied feline Revolution 5-15 lb. Excellent prognosis.
