Medical Summary DVM Intake Exam Estimated age: 8 weeks Microchip noted on Intake? negative History : stray Subjective: BAR Observed Behavior – very friendly and vocal during PE Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS: 5/9 EENT: Eyes clear with OD microphthalmia and mild conjunctivitis but no d/c. OS wnl, ears clean, no nasal discharge noted Oral Exam: kitten dentition, no oral ulcers PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, live fleas noted, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Microphthalmia-r/o congenital issue Fleas Plan: erythromycin ou bid x7d Consider enucleating OD if starts to bother kitten, right now does not seem to bother him Prognosis: good SURGERY: Okay for surgery
