Subjective: Quiet to dull mentation. Rousable
Observed Behavior – Compliant
Evidence of Cruelty seen – none
Evidence of Trauma seen – none
Objective
T =unable to obtain P = 180 R =30 BCS 3/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: Deciduous dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Significant extremely firm fecal material palpable in colon, ascending from rectum for majority of abdomen.
U/G: Male
MSI: Gait not assessed. Prefers to lay on side but able to stand when roused)
CNS: Quiet to dull mentation.
Rectal: Rectal tissue is fluctuant, extremely swollen. Extends 2-3cm beyond tail base. Unable to pass thermometer or Q tip into rectum, unable to express material from rectum. Anus is extremely small, unsure if open/patent.
Assessment
Fecal impaction – suspect anal atresi
Plan
Lily will require surgical correction of suspect anal atresi and fecal disimpaction. Otherwise, her inability to defecate will quickly become life threatening. Immediate placement with surgical consultation recommended
