***GONE 08/05/17***SWEET, YOUNG BOY NEEDS RESCUE ANGEL…Poor LEO has a cold, conjunctivitis and some diarrhea. He is obviously not feeling well and needs someone to give him a home to recuperate. He is only 3 yrs old and friendly.

I came in the shelter as a STRAY on 07/31/2017 from NY 10301, owner surrender reason stated was STRAY.

08/04/2017 Exam Type RE-EXAM – Medical Rating is 4 C – SEVERE CONDITIONS , Behavior Rating is NONE, Weight 5.0 LBS.
Hx: Came in 8/1/17 dehydrated, emaciated, with diarrhea and URI/conjunctivitis. Bloodwork showed neuphilia, hyperglobulinemia, elevated ALT. Started on IVF but dog in kennel under Leo’s chewed out Leo’s catheter in the early morning of 8/3/17. Fecal float was negative for parasites. S: Quiet, hunched in kennel, allows handling O: QAR, estimated 8% dehydrated based on severely decreased skin turgor, MMs pale pink EENT: Mild mucoid crusty ocular discharge, no conjunctival hyperemia today, minimal blepharospasm. Clean nose today. Mild black discharge AU. H/L: Eupnic, quiet lung sounds. NSR, NMA. Abd: Slightly doughy, soft, nonpainful, no masses palpated M/S/I: Diarrhea stuck on hind legs and tail. Amb x4. Emaciated – BCS 3/9. A: 1. Diarrhea – R/O IBD, food sensitivity, lymphoma, malabsorption, CKD, other inflammatory condition (pyelo – not supported by bloodwork), other neoplasia, parasites (prepatent)… 2. Dehydration – R/O secondary to diarrhea only, vs. underlying CKD (not supported with bloodwork) 3. Emaciation 4. URI – appears to be resolving 5. Dental disease 6. Ear mites – on tx Plan: 1. Replace IVC and start IVF at 25 ml/hr 2. Feed I/D 3. Recommend abdominal ultrasound 1088

ACTIVITY LEVEL: Laid back VOCAL: Somewhat chatty CHARACTER TYPE: Calm, Sweet MEDICAL BEHAVIOR: 8/1/17- Subjective: Mews, allows handling. ENRICHMENT NOTES: 8/1/17- Crouched on bedding, occasional meow. Seems sick and miserable. Shuffled forward when door opened, ate treats from my hand, leaned into pets. Sweet, just unwell. EVALUATION: Cage Condition: No change Reaction to assessor: Leo remains neutral, lying down on his cage bedding during the approach. Reaction to cage door opening: Leo remains in place, alert with soft eyes, ears erect and forward. Reaction to touch: Leo slowly head bunts the assessor’s hand and appreciates the petting while he purrs. Reaction to being picked up: Leo remains immobile during the pickup. BEHAVIOR SUMMARY: Average Leo interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

DVM Intake Exam Estimated age: 3-7 years based on overall appearance Microchip noted on Intake? Scanned negative History : Stray, diarrhea, no hx available. Diarrhea in kennel – brown soupy puddles – and on pt’s legs. Ate about 1/3 of canned food this morning. Subjective: Mews, allows handling. Objective Estimated 8% dehydrated based on severely decreased skin turgor. MMs pale pink. BCS 2/9 EENT: Mild mucoid ocular discharge. Mild crusty nasal discharge. Mild brown waxy aural discharge AU. Bilaterally enlarged symmetrical thyroid gland on thyroid slip. Oral Exam: Clean teeth, one canine is fractured PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Non painful, tucked up, no masses palpated U/G: Male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, diarrhea on fur of front legs, hind legs and tail. CNS: mentation appropriate – no signs of neurologic abnormalities CBC/chemistry: Normal Hct. Elevated WBC (25 K/ul), elevated neuts (17 K/ul). Normal T4! Elevated ALT, elevated globulins. Assessment: 1. Severe dehydration, emaciation, diarrhea, thyroid slip – R/O IBD, lymphoma, early renal dz, other 2. Nasal and ocular discharge – R/O URI 3. Fractured tooth 4. Aural discharge – likely ear mites 5. CBC/chemistry: clear signs of infection, but no sign of hyperthyroidism Plan: 1. Place IVC, start LRS IV 9 ml/hr 2. Start metronidazole 0.7 ml PO SID x10 days 3. Start amoxiclav 0.7 ml PO BID x10 days 4. Clean ears and start Tresaderm AU BID x10 days Prognosis: Fair SURGERY: Temporary waiver due to diarrhea and illness. 1088
