Medical Summary DVM Intake Exam 9/2/17 Estimated age: ~6wks Microchip noted on Intake? No History : Stray Subjective: Observed Behavior – QAR, ~5% dehydration. Eating well on own, allows all handling, vocalizes with handling Evidence of Cruelty seen – No Evidence of Trauma seen – Yes Objective P = WNL R = WNL BCS 3/9 EENT: Eyes clear, ears clean, dried crusted bloody nasal discharge noted Oral Exam: Pink mm, teeth erupting normally — right upper canine tooth loose with bruising around gingiva, bruising of gingiva at rostal maxilla PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated; small soft bladder, normal stool in colon U/G: Male MSI: Non ambulatory at RH, unable to ambulate – will fall over in hind end, no palpable fractures but pelvis not symmetric, skin free of parasites, no masses noted, dried blood at right side of face, no wounds CNS: mentation appropriate – no signs of neurologic abnormalities Radiographs: possible right sided pelvic fracture with minimal displacement Assessment: Underage, hind end lameness, R/O right sided pelvic fracture Plan: Continue to monitor while at BACC. Simadol 0.07ml SQ and LRS 10ml SQ administered by LVT Continue with simbadol 0.07ml Sq q24 x 5 days. Prognosis: Fair — may resolved with cage rest SURGERY: Temporary waiver due to underage, injury
