Medical Summary DVM Intake Exam Estimated age: Approx 2 months old History : stray Subjective: Observed Behavior – friendly Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 4-5/9 EENT: OU- conjunctival hyperemia and chemosis, discharge; OS- unable to visualize cornea, partially closed; AU- brown waxy debris; nasal congestion and mild mucoid nasal discharge Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment severe conjunctivitis nasal congestion and discahrge- R/O URI vs other Plan erythromycin OU BID x 10 days doxycycline 8 mg PO SID x 10 days SQ LRS 10 mls x 2 days Prognosis: fair SURGERY: Temporary waiver due to underweight and URI
