DVM Intake Exam 7/1/17 Estimated age: ~ 6yrs, female Microchip noted on Intake? No History : Stray, HBC on highway Subjective: Observed Behavior – Q/BARH. Allows all handling Evidence of Cruelty seen – No Evidence of Trauma seen – Yes Objective P = 160 R = WNL BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate dental tartar/gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Uncomfortable on caudal abdominal palpation, no masses noted U/G: Female – small amount of blood seen from vagina and rectally MSI: Non-weight bearing in hind end, no palpable fractures of long bones, painful on pelvic palpation, small wounds at medial hocks CNS: mentation appropriate – no signs of neurologic abnormalities Radiographs: Lateral and VD pelvic — Right SI luxation, fracture at pelvic symphysis, right hip displaced caudo-ventrally and slightly medially; no other fractures noted Assessment: HBC, right side pelvic fracture, SI luxation, non-ambulatory in hind end at this time Plan: Simbadol 0.55ml given by LVT at presentation. Continue simbadol q24 x 5 days. Monitor eliminations Prognosis: Fair — likely needs surgical repair SURGERY: Temporary waiver due to injuries
