Oral Exam: minimal tartar, some wear
PLN: No enlargements noted
H/L: HR 160 NSR, NMA, slightly pale pink tacky (had vomited prior to evaluating Lungs clear, eupnic
ABD: Vomited brown red viscous fluid when removed from cage. Constipated, doughy abd, cran abd organomegaly
U/G: MN
MSI: Skin tenting, flakey rough coat, emaciated. No overt jaundice
CNS: depressed
Rectal: NE
Assessment:
BG 468, Ketones 3+ r/o dka primary, secondary to pancreatitis, other gi dz
Severe dehydration
Constipated r/o 2nd to dehydration, motility, other
Organomegaly r/o liver 2nd to DM, other (neoplasia, hl)
Plan: RF 24 guage ceph cath 25 ml 0.9% NaCl IV-cat became sternal
25 ml bolus normal saline then added 20mEq/L KCl
Ampicillin 250 mg/ml 0.8 ml IV
9: 30 am Glargine 1 U sq
too dehydrated to obtain chem/cbc samples
Rec rescue asap for dka mgt and further dx
Prognosis: guarded to grave

Mentation: QAR, dehydrated (prolonged turgor and tacky mm, 50ml LRS SQF)
Eyes: clear
Ears: clean
Nose: no d/c
Teeth: moderate staining
If abnormal BCS: underweight, 3.5-4/9 with muscle wasting
Skin: slight jaundice cast around pinnae and face, with erythematous belly (no lesions, no warmth to area, no pain on palp)
Hair Coat: dull but WNL
Declawed: no
Any injuries: no
Behavior: overall dull demeanor, allowed handling and calm but shut down and disinterested in goings on
Medication: N/A, preventatives given
