SAFE 2/1/18 Beautiful LADY MADONNA was a neighborhood cat who was brought to the shelter because someone thought the cat’s ears appeared injured. She has some ear issues and what appear to be self-inflicted wounds on the back of her head. Lady Madonna will need some follow up care for her ear issues. Please adopt this pretty kitty today!

EENT:
-OU: clear
-AS: severe thickening/fibrosis of pinna, canal stenotic but able to appreciate copious amount of severe, dark discharge; pruritic
-AD: severe thickening/fibrosis of pinna, unable to visualize canals due to severe stenosis; moderate amount of serosanguinous discharge present at external ear canal
Oral Exam:mm pk, tacky; CRT <2 sec; moderate tartar, staining; fracture of upper L canine with pulp exposure, tip fx of upper R canine with no obvious pulp exposure
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: female intact
MSI:
-ambulatory x 4 with no lameness
-superficial wounds/excoriations along back of head, likely self induced
-2 small patches of alopecia, erythema over dorsal aspect of L carpus, wood’s lamp neg
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal:grossly normal
Assessment
1. Severe otitis externa with secondary pinnal fibrosis-suspect chronic ear mite infection
2. Wounds along back of head, suspect self induced secondary to severe pruritis.
3. Possible ruptured ear drum AS-r/o fluid from ear canal/pinal inflammation
Carpal dermal lesions, suspect self induced, allergic
4. Fractured canines
Prognosis: fair to good
Plan:
-gave convenia 0.3 ml SQ
-gave dexamethasone SP 4 mg/ml: 0.3 ml SQ
-gave ivermectin 0.09 ml SQ
-baytril 100 mg/ml:0.3 ml Po SID x 10 days
-simbadol 0.3 ml SQ SID x 3 days
-recommend sedated ear cleaning and re-evaluation in 2 days once inflammation may be improved; consider adding topical treatment at that time
-may require bilateral total ear canal ablations due to severe chronic changes
-rec’d dental cleaning with extraction of upper canine
