Basic Information:Lady Macbeth was found in the clients yard as a stray so history is unknown. She is about two and a half months old, black and white DSH female. She was not spayed prior to coming into the shelter.

EENT: Eyes clear, ears clean, mucoid nasal discharge noted
Oral Exam: Teeth erupting appropriately, pink mm, no oral ulcers
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated, distended abdomen
U/G: Female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Underweight, URI, intestinal parasites, FeLV+
Plan: Rec doxycycline 0.25ml PO q24 x 10 days. First dose given
Prognosis: Fair — needs to be retested for FeLV in 4-6 months
SURGERY:
Temporary waiver due to URI, underweight
