GONE 8/15/17 – KITTY was brought in as a possible hit-by-car victim. He has a degloving injury and right hind lameness along with several wounds that are infected. He is also severely anemic and tested positive for FIV. Kitty is very sweet and needs further medical – please help this kitty today!

Medical Summary DVM Intake Exam Estimated age: 5-7 years Microchip noted on Intake? neg; did not place due to BCS History : stray cat, possibly hit by car Subjective: QAR Observed Behavior – a little shy/unsocialized but sweet and allows all handling; good appetite Evidence of Cruelty seen – none Evidence of Trauma seen – RH lameness, degloving injury Objective BCS 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam:mm lgt pk, tacky; CRT <2 sec; tip fracture of upper L canine; moderate to severe tartar/staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: soft, nonpainful, slightly doughy; no palpable abnormalities U/G: male intact, 2 scrotal testicles present MSI: RH-weight bearing lameness with overflexion and guarding of stifle; large necrotic wound over R stifle with purulent discharge severe muscle wasting diffusely CNS: mentation appropriate – no signs of neurologic abnormalities Rectal:not performed CBC-severe, regenerative anemia (10%); mild monocytosis Chemistry-low creat/BUN, low normal albumin despite dehydration (2.2), elevated globulins Assessment Necrotic wound on RH-suspect bite wound FIV positive Diffuse muscle wasting Anemia-r/o secondary to FIV vs other chronic disease vs paraneoplastic Low creat/BUN-suspect due to malnutrition; r/o liver disease (cholesterol, glucose normal) vs other Low normal albumin-would likely be low if patient was euhydrated; r/o PLE vs PLN (kidney disease not suspected) vs wound vs liver disease Dental disease Plan -simbadol 1.8 mg/ml: 0.3 ml SQ SID until placement -sedated with 0.2 ml dexdomitor/0.1 ml ketamine IM for wound cleaning; removed large amount of necrotic skin-early granulation bed forming underneath; several small puncture wounds and/or necrotic wounds noted on medial aspect with purulent drainage; flushed copiously with sterile LRS -radiographs: soft tissue swelling, SQ emphysema over R stifle; significant decreased serosal detail in abdomen; NOSF -gave 100 ml LRS SQ, continued SID x 4 days -convenia 80 mg/ml: gave 0.3 ml SQ Prognosis: poor to guarded due to severe anemia, FIV status and overall poor condition SURGERY: temp waiver due to emaciated condition
