Medical Summary DVM Intake Exam Estimated age: 9-12 months Microchip noted on Intake? neg History : brought in as a stray; was very friendly to finder prior to intake Subjective: bar Observed Behavior – hissing, growling, lunging in cage; examed performed through net Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk; teeth appear very white but unable to evaluate further due to temperament PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female; no spay scar palpable, unable to shave due to temperament MSI: ambulatory x 4 with no obvious lameness or injury but unable to perform full ortho exam due to temperament clean hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Apparently healthy young adult Plan no tx needed Prognosis: excellent SURGERY: Okay for surgery
