DVM Intake Exam
Estimated age: ~5 weeks
Microchip noted on Intake? negative
History : stray
Subjective: QAR, sternally recumbent, ~7-8% dehydrated. Cool to the touch. Minimally responsive. Refuses to eat on own and does not tolerate force feeding very well.
Observed Behavior – very sweet. Easily handleable. Did well for all medical handling and procedures
Evidence of Cruelty seen – no
Evidence of Trauma seen – no

R = eupneic

EENT: Eyes have severe purulent d/c, corneal opacity ou and os is sealed shut with severe chemosis-unable to assess entire globe, ears clean, moderate to severe purulent nasal discharge noted
Oral Exam: kitten dentition, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs have audible upper resp congestion
ABD: Non painful, no masses palpated
U/G: FI, no MGTs, no vulvar d/c
MSI: Ambulatory x 4, live fleas and flea dirt, no masses noted, unkempt hair coat
CNS: minimally responsive
Assessment:
Underage
URI with conjunctivitis
Hypothermia likely hypoglycemia
Not eating
Pale gums
Fleas
Sternally recumbent and minimally responsive
Plan: Continue to monitor while at BACC
Gave nutrical on gums and tried to force feed
Parvo snap negative
Set up in medical with heat support
Cleaned face and eyes
Applied terramycin OU BID
Gave 15ml SQ LRS
Gave capstar 1/2 tab PO once
If no improvement or placement by end of day rec EHR
If improved by end of day start doxycycline 10mg/kg PO SID x10d, terramycin OU BID x10d, SQ LRS 20ml/kg BID x3d, nutrical BID PO x3d, heat support PRN, and daily rechecks
Prognosis: guarded
SURGERY: temporary waiver from surgery due to illness
