Medical Summary DVM Intake Exam Estimated age: 4-7 years based on dentition and overall appearance Microchip noted on Intake? No History : Stray, no medical hx given Subjective: Weak, doesn’t move very much, allows all handling, kneads paws while being cared for Objective BCS 2/9, MMs pale pink, decreased skin turgor, estimated 8-10% dehydrated EENT: OD cornea is not visible – just thick bulging conjunctival tissue with a large deep red crust on it. There is some serosanguinous fluid leaking from the eye, plus mild mucoid discharge. OS mild mucoid discharge. Clean nose and ears. Oral Exam: Thick purulent discharge on the right side of the mouth, pt resists oral exam. Possibly missing some teeth, tartar on others. PLN: Right mandibular LN is thickened H/L: Tachycardic, grade 2/6 systolic heart murmur. NSR. Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Weak but appears to be ambulatory x 4, no injuries noted. Thick flea dirt. No masses noted, healthy hair coat. CNS: mentation appropriate – no signs of neurologic abnormalities FeLV positive Assessment 1. FeLV positive 2. Right eye – possibly ruptured or severely infected, likely fistulated into oral cavity 3. Heart murmur – R/O due to anemia and emaciation vs. underlying heart disease 4. Conjunctivitis 5. Severe dehydration and emaciation 6. Flea dirt Plan 1. Administered 200 ml LRS SQ during exam 2. Run CBC/chem/lytes/T4 3. Amoxiclav 0.7 ml PO BID x10 days 4. Baytril 22.7 mg tabs, 0.5 tab PO SID x10 days 5. Erythromycin OU BID x10 days 6. LRS 150 ml SQ SID x4 additional days 7. Will likely need enucleation Prognosis: Fair to guarded – depends on results of bloodwork SURGERY: Temporary waiver due to poor health 1088
