08/12/17 S/O: QAR. ~5% dehydration. Appetite OK (ate most of wet food). Clean litter. Allows all handing. BCS 3/9 EENT: Mild dental tartar/gingivitis, no oral lesions noted, nuclear sclerosis, iris atophy OU, no ocular or nasal discharge, abrasion on nose, dermatitis at chin HL: Normal thoracic auscultation ABD: Normal abdominal palpation INTEG: Slightly unkempt hair coat, ulcerations on paw pads and few digits, superficial wounds/ulcerations at right antebrachium and both hocks, very slighty crusting at ear margins; no other lesions noted at this time MS: Ambulatory x 4 UG: Female CBC shows mild leukocytosis chararcterized by a very mild lymphocytosis and basophilia and moderate eosinophilia. Chemistry shows mild globulinemia. All other results including T4 within normal limits A: Geriatric, underweight, mild dehydration, skin disease – R/O eosinophilic granuloma complex vs neoplasia vs other P: Rec LRS 100ml Sq q24 x 3 days, conveniia 0.3ml SQ. Rec allergy trial and strict flea treatment regimine with placement, consider course of steroids. Good short term prognosis WBC 20.92K/uL (2.87-17.02) LYM 7.96K/uL (0.92- 6.88) EOS 3.99K/uL (0.17- 1.57) BASO 0.30K/uL (0.01- 0.26) GLOB 6/3g/dL (2.8- 5.1)

08/09/17 20:15 Kelly is a medium white and black cat. She came to the center as a stray. During intake, she was very vocal in her carrier. When counselor went to remove her from her carrier, she was very tense and grasped onto the counselor’s arms. Counselor set her down and she walked back in her carrier. She allowed some petting but did shy away after a few seconds.

08/12/2017 BEHAVIOR EVALUATION – EXPERIENCE

KNOWN HISTORY: Kelly was brought in as a stray, so we cannot speak to her behavior in her previous home. MEDICAL BEHAVIOR: Timid, but allows examination and handling with minimal restraint. vocalized a little during exam ENRICHMENT NOTES: 08/10/17 Lying in back of kennel with a relaxed body, head resting on kennel floor. She allows petting along her head and body while leaning in gently. Sweet older girl. Doing well. EVALUATION: Cage Condition: Cage recently cleaned Reaction to assessor: Kelly was curled up tight with her head lowered underneath her kuranda bed. Reaction when softly spoken to: Kelly lip licks, turns her head away and faces the wall. Reaction to cage door opening: Kelly remains motionless. Reaction to touch: Kelly starts breathing more rapidly and curls up tighter when the assessor extends his hand out. She seems unsure at first but starts to open up with touch. After a couple minutes of attention she leans in for rubs then raises her rear and rises up. She slowly starts to build confidence the more you interact with her but she tends to stay near the back of the cage where she feels comfortable. Reaction to being picked up: Kelly was tense when picked up, clung onto the assessor’s chest then leaped back into the cage. ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Timid RECOMMENDATIONS: Experienced cat parent – may be a little more independent, and may need time to warm up to her new home. We recommend that this cat go to a home with experienced cat parents.

Oral Exam: mild tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, Coat is good for most part. Pinna have scabs, secondary to scratching. carpal pads are red/inflammed (secondary to carrier trauma?). Small area of pigmentation/alopecia lateral antebrachium (both left and right): secondary to overgrooming? CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Thin Evidence of scratching/pruritis: rule out inflammatory/allergic vs infectious vs other Evidence of face rubbing: rule out allergic/inflammatory vs infectious. Plan: vaccinate microchip deworm flea tx combo test given 0.3 ml convenia SQ monitor for pruritis, continued dermatitis and facial rubbing. Consider scrapes and cultures if progresses. Suspect allergic dermatitis Prognosis: fair to good depending on dermatitis SURGERY: Permanent waiver due to age
