Medical Summary DVM Intake Exam Estimated age: ~9-10 years Microchip noted on Intake? positive 0A106C6406 History : o/s, adopted from ACC in 2008 Subjective: BARH Observed Behavior – Nervous and very vocal but did not try to resist medical handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge Oral Exam: mild tartar, adult dentition, no oral ulcers noted but limited oral exam due to behavior PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FS MSI: Ambulatory x 4, lots of flea dirt noted but no live fleas, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Fleas Geriatric Dental disease Plan: Continue to monitor while at BACC Prognosis: good SURGERY: spayed
